Patient, who was diagnosed with focal segmental glomerulosclerosis (FSGS) two years ago after undergoing kidney biopsy due to symptoms of epistaxis, is currently undergoing treatment with six medications including sustained administration of Felodipine. Regular physical examinations are conducted every three months to monitor renal function indicators.
Two years ago, the 24-hour urine protein level was 1.4g. Recent blood tests revealed 3+ urine protein and elevated neutral fat levels at 341mg/dL.

There are no issues with the administration of Felodipine and Allopurinol. To address the high urine protein situation and delay the decline in renal function, I propose adding medications. Among them, I particularly recommend Fosinopril as a medication to delay the decline in renal function. Additionally, as blood lipids (neutral fat) are also elevated, I suggest adding medications to lower blood lipids.
I recommend adding three types of prescription drugs:
Antihypertensive drug to reduce urine protein: Losartan Potassium 25mg
EPA preparation to reduce blood lipids: Rotoriga 2g
Medication to protect renal function: Fosinopril
Treatment Method
The main approach to treating FSGS in Japan is to reduce urine protein, protect the kidneys, and stabilize blood lipids. Complete cure of FSGS is difficult, but the main goal is to slow down the progression of kidney function. Weight management and moderate weight loss are also recommended.
